We have a standard notice on our statements that says,
"The total balance on your account is $XXX.XX. Of this amount, $XXX.XX is your responsibility and is due in full at this time. If you have any questions about the way your insurance company handled your claim, please call them at the customer service number on your insurance card."
This puts the emphasis on the fact that it is the patient's claim, the patient's responsibility, and the patient's insurance. Sometimes if I have the insurance number handy, I will add that to the individual's statement (such as the number for Medicare).
